(Pete Hueseman's 4th visit to Power Surge) Dearest: It's always a pleasure for me to welcome back Power Surge's Pharmaceutical Consultant, P E T E H U E S E M A N Pete has been a registered pharmacist for 25 years and exclusively involved in the area of women's health for the past six years Pete brings his expertise and caring to Power Surge on behalf of his pharmacy, Bellevue Pharmacy Solutions, a pharmacy which compounds prescription natural hormones, such as progesterone, estradiol, estrone, tri-estrogen, DHEA, testosterone, pregnenolone and other compounds as well. Pete Hueseman is professionally associated with NAMS - the North American Menopause Society the Academy of Alternative Medicine, Alternative and Complementary Medicine Association- and is registered with various professional pharmacist's associations. Pete, welcome back to Power Surge :) Pete Hueseman: It's great to be back. Dearest: Pete, is there any one particular protocol that's more popular among the women you work with? Pete Hueseman: I do believe that the Biest & progesterone combination is more popular now. The biest is gaining on Triest! Dearest: Can you share with those who don't know exactly what bi-est is and tri-est? Pete Hueseman: Biest is 80% estriol & 20% estradiol and Triest is 80% estriol & 10% estradiol and 10% estrone. Dearest: Under what circumstances would you recommend one over the other? Pete Hueseman: One thing I did learn in the last 2 weeks is that estrone is not necessarily bad for one, I recommend the biest when ones symptoms require a stronger estrogen to relieve the symptoms. Dearest: You find that bi-est is more effective than taking, say, estradiol alone? Pete Hueseman: Not necessarily, but many women like the estriol along with their estradiol. Dearest: Does this provide more of a hormonal balance than taking just estradiol? Pete Hueseman: I think it does help balance the system better, yes. Dearest: Thanks, Pete Let's go to our queue now. Castle, go ahead with your question for our guest :) Castle 812: Does estradiol have the same effects as estriol? Pete Hueseman: No, estradiol is about 8 times stronger, therefore you need a smaller # for the mg dose. PianoMary: Pete, I know you've helped lots of women in Power Surge with their hormones Do you also work with other medicines such as those for arthritis? Pete Hueseman: Yes, at Bellevue Pharmacy we work with all diseases and conditions, we have a full line of products. PianoMary: Thanks Dearest: What, for example, would you prescribe for a menopausal woman with arthritis? Pete Hueseman: I would have to know more about a particular condition, but we do lots of ibuprofen, and our penetran ointment to rub on as well as some special indocin, lidocaine, prilocaine gels and creams. Longtime03: Thanks for being here, Pete What, if anything, can you tell us about soy-based natural HRT? Pete Hueseman: It works well, it is the safest form to use as far as side effects and smaller incidence of causing cancers, usually tolerated quite well by most. MAMAdee610: Can premenopause cause constant dizziness that would have the same symptoms of having stress and anxiety? My doctor put me on Xanax and blood pressure pills. Pete Hueseman: Yes, it could, this is a sign that you need to balance your hormones properly with probably natural progesterone as well as natural estrogen (but make sure it is not do to high blood pressure, too). Dearest: Pete, what are your thoughts on estrogen dominance? Pete Hueseman: Estrogen dominance can be awful, but we can help people with that by prescribing a little stronger dose of progesterone, exercise and a correct diet. Dearest: What's the usual dose of progesterone you recommend? Pete Hueseman: Progest 200mg sublingual tablets 2 times a day on days 13 thru 27 of the cycle, may use 3 a day on days 20 thru 27 if necessary. Dearest: Thanks, Pete :) DHu107, go ahead. DHu1070513: Are equal amounts of progesterone cream & sublingual tablets absorbed equally? Pete Hueseman: No, the sublingual tab is absorbed the best, 2nd only to a rectal suppository, you lose about 50% going through the skin. Dearest: Pete, someone told me that when progesterone is taken orally that a large amount is lost in the first liver pass Is that true? Pete Hueseman: That is true, that is why we developed the sublingual tab about 10 years ago, since the suppository isn't to pleasant to use, and the creams weren't studied yet. Dearest: Interesting. Thanks VpsIII, go ahead, please VpsIII: I have a question about DES, which is not natural Do you know the half life of DES, and as a perimenopausal woman should I avoid all estrogens? Pete Hueseman: It is not used any more except mostly in animals, and I don't remember the 1/2 life of it, but it is long; and I do not believe that you have to avoid all estrogens in the future, but you must be careful of them. Sja4home: Is there a preferred recommendation for a young(er) woman (mid-30's) who has many years of replacement ahead of her It would be strictly for hormone replacement (post meno with no meno symptoms at all) Pete Hueseman: Natural is always better, but if you have something specific in mind, please call me at work and we can go over it. JPLJAXX: Can compounded prog 10% methyltes 5mg/ml cause staining between periods? A cause for concern? Pete Hueseman: It could, yes, have you been checked out internally by your doctor, the doses could be wrong and need adjusting if there is nothing else wrong, and therefore it may not be anything to be concerned about. JPLJAXX: Have an appointment in 2 weeks to review results of saliva study and internal evaluation. Thanks - guess it needs to be checked out! Pete Hueseman: That is a good idea. Dearest: Pete, is it true we get a clearer reading of progesterone levels through saliva testing than blood tests? Pete Hueseman: I do not believe that is necessarily true, most doctors prefer the blood test, but the saliva does work too. Proftack: Do you recommend any compounds that don't cause migraines? Thank you! Pete Hueseman: Hopefully all my recommendations do not cause migraines, but sometimes it does happen, thankfully not very often, if it does this is a sign that you should communicate back to me & your doctor to fine tune or change the meds. Pete Hueseman: Yes, I believe that the estradiol could replace it, and if you would like I could give you a suggestion of what amount of estriol to add to it too--call me this week at work. typically it would be estradiol 2mg ( maybe with estriol 4mg along with it). Mahj Mar: Thank you, I will call JILLY: What has been the feedback on the new combi-patch? Pete Hueseman: I haven't heard much about it since September, we don't use very much of it yet, the progestin in it is still synthetic, and that is usually what causes the problems. Longtime03: I had surgical menopause 4 yrs ago; have experienced severe menopausal symptoms since My Dr. changes meds each visit (yearly); I'm now on Premarin 250. It stops most symptoms, but has lots of side effects & kind of scary Pete Hueseman: Sounds like you need to try our 3 in one sublingual tbs, (progesterone & estradiol & testosterone and use it under the tongue twice daily--excellent absorption & results with very little side effects you definitely need to call me and see if your Dr. will let you try our sublingual for a month or two. Longtime03: Thanks, Pete--will do Dearest: Pete, under what circumstances *shouldn't* a woman take any form of HRT? Pete Hueseman: Probably only if she is in the beginning stages of breast cancer, I do believe I sent you a new article to post about using estrogens even with breast cancer, new study out just last summer. Dearest: Only in the beginning stages of breast cancer? Is it your opinion that it's okay if she has a family history of breast cancer? Pete Hueseman: Yes, if the lady is totally uncomfortable a low dose of natural is ok, but go to the Dr. every 6 months, and do self testing frequently also, be sure to use it with natural progesterone. MT1316: Do you have anything better than Prempro, does not seem to help Pete Hueseman: Yes we make a natural Prempro it is (progesterone 100mg or 200mg plus estradiol 5mg or 1 mg ) all in one oil capsule and you take it once a day at bedtime it works wonders for many thousands of women. MAMAdee610: Hormone test was ok on zanex and lopressor for palipationg dizziness is great with clogged ears palpitations sweats said it could be pre menopause but dizzy when the zanex wears off Pete Hueseman: I would go see a ENT specialist, or a cardiologist to be sure it is not something else, but if all is ok, you may still need a dose of natural estrogens with progesterone to solve it. MAMAdee610: Have done that, brain scans also Will that help the dizziness Pete Hueseman: Please call me at work this week. TogaGirl4U: How does progesterone creme and sublingual tablets compare for absorption? Are 100 mgs of each the same absorption? Pete Hueseman: Yes, to balance your hormones can help anxiety and dizziness oftentimes, and No, you would use more cream to get the same results typically. TogaGirl4U: How much more? Pete Hueseman: 50mg sublingual tabs would work like 100mg/gram of cream. JPLJAXX: Should application of compound cream necessarily be scheduled according to cycle? Is daily too much? Possibly cause staining? Pete Hueseman: You would use it cyclically if you were still cycling, but could use it daily if you were post menopausal, if post -menopausal and you still stain, then the dose should be adjusted upward during the time the staining occurs. JPLJAXX: Shouldn't be using daily if not post menopausal? Pete Hueseman: No, daily is not necessarily, too much. JPLJAXX: Even if not post menopausal? Pete Hueseman: Probably not JPLJAXX: OK - thanks Sja4home: What is the name of the 3-in-1 sublingual tabs so I can ask my doctor about it? Sounds intriguing! If it's only available through you, how can I get more info to bring to my endocrinologist? Pete Hueseman: Progesterone 100mg & estradiol (or biest) & testosterone all in one sublingual tab --no short name sorry. Sue: Are you finding more Doctors are understanding the role of the progesterones even for surgically meno women? Pete Hueseman: Yes, the progesterone helps to balance the entire system, it has many benefits besides protecting the non-existent uterus yes, the doctors are becoming believers! Mom350: Have you heard of Maca, herb to help with the libido? I want to enjoy sex again!! Need information, thanks Pete Hueseman: I do not know about Maca, but we do sell a lot of testosterone in the natural form for that, and it works well safely! call me with your name & address & I will mail it. Dearest: Pete, what tests should a woman take before determining if she should take hormones? Pete Hueseman: I would test a baseline of estradiol, estriol, progesterone, testosterone, dhea. Dearest: Thanks Proftrack, go ahead, please Proftack: What is most used compound for night sweats, lack of sexual desire, and protection for bones, heart, etc? Pete Hueseman: Triest, Biest or estradiol, along with natural progesterone, and sometimes some testosterone, for those of you taking your daily dose of estrogen in the AM and still having night sweats, try moving to after supper or at bedtime and see if it works better. Wcmarie2: Is there anything I can do to lessen the effects of hair growth to the local skin area with 1% testosterone. I currently am taking triest 25 with 25 mg progesterone Pete Hueseman: Use a lower dose, 1% is 10mg and most women only need 25 mg per day. Wcmarie2: By the way, the effects are wonderful Pete Hueseman: I still think you need to lower the dose, and get rid of the hair problem. JILLY: Is Prometrium the same as what you use? are they both absorbed the same? Pete Hueseman: Prometrium is progesterone 100mg in the oil capsule (peanut oil) it can cause many allergies, we use the safflower oil instead and have many less problems, we can also make it in a 25mg 50 mg 150mg or 200 mg capsule and yes they are absorbed the same way. Dearest: Pete, how are your prescription "naturally compounded" hormones different from using phytoestrogens, aka soy? Pete Hueseman: They by the way are processed from the soybean plant, and come out a pure prescription powder- very potent, the phytoestrogens must be absorbed in the body and attach to the estrogen receptor sites to show some estrogen like activity to then be of benefit they are much milder. Dearest: Even if they contain very large amounts of soy isoflavones? Pete Hueseman: The larger amounts that they contain, the better they seem to work but it is the same process. Dearest: That's what I wanted to know. Thanks :) Mom350, go ahead. Mom350: If I’m drinking revival do I also have to take estrogen in another form, what about progesterone Pete Hueseman: Your Revival is strong and seems to help many women (and men). Dearest: Yes, Revival has helped many women :) Pete Hueseman: Mom, it depends on your symptoms if any, you could call me to discuss it in depth. Mom350: Do I still have to take progesterone? Lizybear10: I have been taking 3mg of sublingual estrogen, all of a sudden it is not enough insomnia is back and flashes and anxiety Pete Hueseman: What kind of estrogen? estriol, estradiol or estrone or what combination, where do you get it from? sounds like you need a dose adjustment though. Call me. Lizybear10: From you. Why would it change all of a sudden? Pete Hueseman: Call me at work tomorrow so I can review it! Yes it could change all of a sudden in some cases changes could be do to sudden stress, illness, other meds etc. Catalina5: How effectively is transdermal progesterone absorbed by most women? Pete Hueseman: It usually works well, but it takes lots of effort, and sometimes is inconvenient to apply while at work, etc., but once you get the right dose figured out many really like it. Dearest: Great info, Pete. Thanks. Castle, your question? Castle 812: I understand that there have been new studies that document that estradiol is as safe as estriol in respect to cancer ? Pete Hueseman: It is safe for the most part, and much more beneficial to the bones, heart and vaso-motor symptoms, we do have a new study published last summer that supports this. Tried estrace with micro progesterone, vivelle plus prometrium and now combipatch Estradiol is still low at 50!
